  • Color Copy of All Owners Driver’s Licenses (front and back on separate file)
  • Payroll Documentation
    • 940 Form (2019)
    • 944 Form (2019)
    • 941 Form (up to all four quarters of 2019)
    • 1040 Schedule C (2019)
  • February 2020 Business Bank Statement

Complete the quick form and you will be directed to our partnering lender's portal to apply for the PPP loan.  Once you complete your application check your "Inbox" for an activation link to complete tasks such as uploading payroll documents.  Once tasks are completed, you application will be submitted directly to the SBA (on hold until the PPP is funded the second time by the government.
